| Change of Address | Complete this online form to keep the MBA office updated on your address. Also go to MyUW and update your address with the University. |
| Independent Study Request (pdf) | Use Independent Study 600 to pursue subjects or topics not offered in a regularly scheduled class. Review the Elective Registration Handbook for further information about independent study. |
| Master’s Degree Request | Complete online form for graduation, if graduating this quarter. See Graduation for dates and information. |
| MBA Lounge Access (pdf) | This form is required to code your Husky ID card so you can use it as a key card to open the MBA Lounge doors. Dates for scanning ID cards are arranged by the MBA Program Office. Specific dates are announced in the MBA Newsletter. |
| MBA Scholarship Application | Online scholarship application. Read about MBA scholarships first! |
| Non-MBA Class Request Form (pdf) | Use this form if you wish to take a class outside the MBA Program and count it toward your degree requirements. See a list of pre-approved non-MBA classes not requiring this form. Submit this completed form to the MBA Office at least one month prior to start of the quarter. Approval takes about a week and is required before you start the class. |
| Online Pre-Registration Form | Complete this online form to bid on electives. For more information visit the Elective Registration page. |
| Professional Development Activity Notification (pdf) | Complete this form and provide the MBA Program with documentation on any activity not included in the regular MBA Program that you wish to have considered to count toward your professional development (practical experience or international perspective) requirements |
| Registrar's Office Forms | The UW Registrar’s Office site includes access to assorted forms for registration, student life and records such as consent forms and refund applications. |
| Request Form for Practical Training Letter (pdf) | Submit completed form, along with the ISO Work Authorization form to the MBA Program Office, allowing 2 day-turnaround. For more details on Practical Training, visit the International Student page. |
